Community Impact Process Opens

The United Way of Bay County is pleased to announce the release of the 2018 Community Impact Grant Initiatives. The grant process is a two phase process, with the first being an application of interest in submitting a grant request. It is open to all qualified 501 (c) 3 health and human service providers serving in Bay County.

January 10, 12, 17 & 19, 2018

Community Impact Informational Sessions

Learn about the application process, strategies and initiatives, measurement tools, and scoring rubric. Sessions will be held in the afternoon on January 10 and January 17, 2018 at 1:30pm – 3:30pm and in the morning on January 12 and January 19, 2018 at 8:30am – 10:30am. All sessions will be held at United Way of Bay County in the Community Conference Room.

February 14, 2018

Phase One proposals Due

Phase One consists of submitting a brief application, an overview of the programs logic model, and required organizational governance and financial information. The submission must identify which of UWBC’s Community Impact Areas the program addresses and how the program intends to create measurable impact in an UWBC’s 2018 Impact Initiative.

March 1, 2018

Community Review of Phase One Proposals

If all Phase One requirements are met, the submissions will be distributed to the Community Impact Committee for review. The team will meet to evaluate each submission for the program’s alignment with the UWBC Impact Initiative and Strategies for the respective Impact Area and the program’s ability to produce significant impact in the live of their program participants.

March 31, 2018

Phase One Applicant Notified of Status

After Review, the Community Impact Committee will forward their recommendations for advancement or denial to UWBC staff. Programs that are recommended to move forward will receive an invitation to submit the Phase Two of the application process.

May 1, 2018

Phase Two Proposals Due

In the Phase Two of the application process, applicants will list two or more of the program’s short term outcomes as submitted in the Phase One application. Applicants will be asked to elaborate on the outcomes and include an evaluation plan for each. Additional organizational governance and financial information will be required to attach.

June 1, 2018

Committee Review of Phase Two Proposals

If all Phase Two requirements are met, the submissions will be distributed to the Community Impact Committee for review. The team will meet to evaluate and prioritize each submission based on the program’s organizational capacity to deliver outcomes, sustain operations, establish continuous improvement initiatives, remain accountable, produce measures and be transparent.

June 28, 2018

Recommendation for Community Impact Awards to UWBC Board

After review, the Community Impact Committee will submit their recommendations of program to receive funding to the UWBC staff.

July 1, 2018

Committee Impact Funds Awarded

After receiving UWBC Board of Directors approval, all applicants will be notified of their award status before formally announcing the 2018 award cycle recipients.

January 31, 2019

6 Month Outcome Reports Due

Choose 2-3 Short-Term Outcomes as included in the program’s Logic Model from Phase One application which you intend to measure and select 1-2 Long-Term Outcomes as included in the program’s Logic Model from the Phase One application which you intend to measure.

July 31, 2019

12 Month Outcome reports Due

Choose 2-3 Short-Term Outcomes as included in the program’s Logic Model from Phase One application which you intend to measure and select 1-2 Long-Term Outcomes as included in the program’s Logic Model from the Phase One application which you intend to measure.

January 31, 2020

18 Month Outcome Reports Due

Choose 2-3 Short-Term Outcomes as included in the program’s Logic Model from Phase One application which you intend to measure and select 1-2 Long-Term Outcomes as included in the program’s Logic Model from the Phase One application which you intend to measure.
